  • Full name of the individual being searched for
  • Birthdate of the individual being searched for (i.e., month/day/year)
  • Last four digits of their Social Security Number or Individual Taxpayer Identification Number (ITIN). If you do not have this information, select “I don’t know” and enter “000-00-0000” as your answer to that question.   • Type (RN or LPN) and class (I or II) depending on whether they were issued before July 1, 2017; if they were issued after that date, there won’t be classes listed next to either type since they don’t exist in Arizona anymore!   • Go to the Arizona Online Services page.
  • Click on the Arizona license lookup option that you want: RN or LPN.
  • Enter your name, birth date and social security number; then click “Continue.” If you do not have a valid Arizona driver’s license or state identification card with your current address, enter any other form of identification that proves who you are and where you live within the state of Arizona (such as a passport).
